Hospital Discharges

Proper planning at the time of hospital discharge is very important.  If the patient has the right equipment and care arranged, the chance of re-admission will be minimized.  Home health agencies normally provide “limited” visits after discharge while other agencies, family or friends provide the ongoing care.  Private duty agencies usually provide more of the continuous care depending on the need of the patient.  Celestial Care has developed a specific hospital discharge protocol that we use with the hospital case manager or social worker to insure a safe discharge.  Further, we coordinate with the medical supply companies to make sure all the necessary equipment is delivered to the home prior to discharge.
